Pet Registration (First Pet)

All residents are required to register their pets with the management office and must be approved by the Board of Directors prior to pet occupying any unit. Upon completing
registration and being approved, each pet will be assigned a red pet tag which must always be worn by pet when on the premises of Four Midtown Miami.  Along with the pet registration the following items must be emailed to communications@4-midtown.com:
a) Recent picture of pet
b) Updated Veterinarian vaccine records, including rabies records/certificate
c) Proof of Pet-Liability Insurance

By registering your pet, you agree to the 4 Midtown Pet Policy.
